This can be your business license, State Sales tax certificate, etc. Or, you may use our Missouri Sales Tax Exemption Certificate to send us your information. On this form, complete the highlighted areas, include your email address, and email the form by clicking Submit. If you prefer, you may print the form, sign it where indicated and, fax it toll-free to 888-424-3294.

BE SURE TO INCLUDE YOUR EMAIL ADDRESS. Once the form is received and your account approved, you will be emailed the Wholesale Price List.

We ship via the least expensive method (UPS or commercial truck). If you do NOT have access for commercial truck deliveries (Not UPS type), please indicate this when you submit your forms.

Established Wholesale customers may process orders through the shopping cart however, the prices in the cart will NOT reflect your discount. Simply type WHOLESALE in the Special Instructions box and your discount will be applied when your order is processed provided we have your Proof of Business information on file. If you have any questions, contact us via email.

We ship via UPS or USPS so be sure to provide an appropriate street address including any suite, apartment, or unit number. These charges are included in the order total in the checkout process. If you there are any special instructions for your delivery, please indicate this in the "Special Instructions" box during the checkout process. Please double check your billing and shipping address. There is a $14.00 address correction fee imposed by UPS that will be charged to your credit card if the address you provide is incorrect.
